Eligibility:
Available to people aged 16 and above whose ability to manage their daily activities and to live independently in the community is severely impacted by mental illness. Participants must reside within the Eastern Hume service area.

About this service:
The PHaMs Program for the Eastern Hume Region is a Commonwealth Government funded initiative. It is a strengths based, recovery oriented service working with people who are isolated from their community, either geographically or socially. Community Support Workers and Recovery Mentors undertake individual outreach from offices in Wodonga and Wangaratta. The service aims to assist people to develop the necessary skills and confidence to be successfully linked with their communities and the various services they require.
