Eligibility
All age groups - people with diabetes / pre-diabetes or their carers. Self and GP / Specialist referrals are accepted for anyone who has diabetes or pre-diabetes. Relevant information needs to be sent or obtained for all referrals, this includes: reason for referral, recent and past medical history; current medications; relevant pathology results e.g. FBG, OGTT, eGFR, cholesterol

Diabetes - Individual Consultations and Education
The Diabetes Educator provides one-on-one consultations for person with diabetes / pre-diabetes and also their carers /parents. This includes:
• An initial Assessment and future re-assessments as required. The assessment includes obtaining information from the person’s GP or Specialist to help develop a management program;
• Developing a self-management plan with the person to develop individual achievable goals and setting planned actions;
• Provision of education in relation to the latest diabetes evidence based care guidelines (this includes pre-diabetes prevention management);
• Review and monitoring of the self-management plan.
• Group education
The Diabetes Nurse Educator works collaboratively with other service providers where required, such as GP’s, Endocrinologist, Dietitian, Podiatrist, and many others, to give you the best possible chance of maintaining optimal health. Communication between service providers is important, especially between a person’s GP or Specialist in relation to any significant changes or planned actions.
Outreach service In a collaborative partnership with Castlemaine Health the staff from the hospital may refer inpatients to the Diabetes Nurse Educators for review and assessment. This enhances seamless transition from hospital to home.
Diabetes Nurse Educators also provide education inservices to hospital staff on a regular basis.
Schools Families and schools can request visits by the the Diabetes Nurse Educators to provide education and information to teaching staff and student groups.
Prisons Prison staff request individual or graoup education sessions for staff and/or inmates as needed.
