Eligibility
This program targets people (all age groups) living with asthma or other lung diseases such as C.O.P.D. A referral from your GP is required for this service. Referrals need to include relevant information such as history of smoking, exposure to smoke, dust or other environmental factors, etc. It is essential that all GPs provide an information pack to their patients as part of the referral process.

This program is designed to assist with diagnosis and optimal treatment of Asthma and other lung disorders such as C.O.P.D.

TO ARRANGE AN APPOINTMENT CONTACT: Reception
 Drop in or phone our reception desk and ask for an appointment.
 The receptionist will take your name and phone number and ask you a few very basic questions.
 You will be given a date and time for your appointment.
 You will be given an information pack that outlines what you need to do on the day of the appointment.
