Asthma and Anaphylaxis Education

Eligibility
This program targets the general community, and people (all age groups) and their carers / families living with asthma. You need to have been diagnosed as having asthma, but you do not need a referral. Please bring along or provide the following information if a referral is made: recent and past medical history, stability of issue, supporting documentation, medications list etc – anything to assist the educator.

Individual Asthma and Anaphylaxis Education
Our Asthma Educator at CHIRP provides a one-on-one consultation for adults and children that includes an assessment, education, and Asthma Management plans.

This program is designed to provide information and education to assist people/families living with asthma to understand and manage their asthma/anaphylaxis. It assists the person with asthma/anaphylaxis to feel in control and to maintain a sense of well-being.

The first appointment is a 60 minute consultations. Following sessions will be shorter (depending on need).

The Asthma Nurse Educator works collaboratively with other service providers where required, such as GP’s, Physicians and many others, to give you the best possible chance of overcoming whatever problems you may have. Communication between service providers is important, especially between a person’s GP or Specialist in relation to any significant changes or planned actions.

 Drop in or phone our reception desk and ask for an appointment.
 The receptionist will take your name and phone number and ask you a few very basic questions.
 You will be given a date and time for your appointment. sr
