The Service Coordination Best Practice and Continuous Improvement Manual is an outcome of the Western Metropolitan Region Cross Alliance collaborative approach to undertaking a series of 'Whole of Region' funded projects. This First Edition of the Manual is intended as a comprehensive resource for agencies/service providers to facilitate the implementation and sustainability of the Best Practice and Continuous Improvement approach to the Service Coordination reform.
The Manual is designed to assist agencies make the practice changes necessary to implement Service Coordination in a consistent manner and to support the implementation of the Statewide Service Coordination Tool Templates.
All primary care agencys are welcome to use the WMR Manual for the purposes of supporting the implementation of service coordination reforms. All use must be not-for-profit and should appropriately attribute the WMR PCPs in any publication. Any for-profit use should be negotiated with the Department of Human Services.
In October 2001, the Western Metropolitan Region (WMR) Primary Care Partnerships (Brimbank-Melton, Moonee Valley-Melbourne, and Westbay) jointly received funding from the Department of Human Services (DHS), based on a comprehensively prepared submission, to develop and implement 'Whole of Region' Service Coordination Projects.
Throughout 2002, the PCPs' member agencies worked in partnership with high commitment, resourcing, energy and enthusiasm in the development of this Best Practice and Continuous Improvement Manual for Service Coordination. A fortnightly series of workshops and forums provided a structure through which the practice, process, protocols and systems were identified and documented. Deb Warren, community services consultant, coordinated and developed this first version of the Manual.
