Connectingcare was originally funded through the 'Networkng the Nation' funding stream. The Victorian Department of Human Services (DHS) funded the development of the PKI encryption capability in 2002. Further funding has been received through the Hospital Admission Reduction Program (HARP) to develop basic integration with the iSoft Clinical Intranet.
Since 1997, Connectingcare has been meeting the service coordination needs of intake staff, managers and health professionals across Victoria.
Over one hundred primary care, community health and acute agencies use our services directory to find the most up-to-date information on over 6000 services across Victoria.
Our e-referral system enables client information to be sent and received using encrypted email. We use existing Internet-based technology to create a secure platform for referral without the cost of additional software or equipment.
Connectingcare is a not-for-profit project auspiced by ISIS Primary Care. Strategic management is provided by a User Group of representatives from Primary Care Partnerships registered with Connectingcare.
